Story recap: Nightmares of Today

Primary Reason for Therapy: David is undergoing therapy for severe depression, post-traumatic stress disorder, and substance abuse. His condition has deteriorated, with increased isolation and heightened paranoia. He continues to struggle with forming relationships and has trust issues. I am his fifth therapist. The trauma from his kidnapping at age 11 remains unresolved and appears to be deepening.

Note on margin: I sense a growing detachment in David. His stories don’t always align. Could there be something else under the surface?

Current Issues: David reported persistent paranoia, describing feeling as if he's being watched or followed. His nightmares have intensified, and he’s begun hearing fragments of conversations he can’t explain, leading to heightened fear and confusion. He also mentioned increasingly vivid flashbacks, often triggered by minor encounters.
Mood & Affect: Highly anxious, paranoid; flat affect with moments of agitation. Frequent dissociation during the session.

Client’s Progress: David has stopped using grounding techniques altogether, expressing a belief that “nothing helps anymore.” He has not adhered to his medication routine, reporting that he "doesn't trust it" anymore, fearing it makes him more vulnerable.

Interventions Applied: Cognitive-Behavioral Therapy was attempted to challenge his paranoia, but David remained resistant, questioning the reality of the situation. Grounding exercises were reintroduced but rejected by David, who claimed that he "can’t focus" when the anxiety hits. Discussion on medication’s side effects and changing everyday environment was brought up, but David remained skeptical.

Primary Reason for Therapy: David is seeking therapy for severe depression, post-traumatic stress disorder.. Problems with engaging in relationships, risk of substance abuse. Trauma seems to stem from the kidnapping at age 11.

Current Issues: David reported increased anxiety and nightmares related to his past trauma. He expressed fear of falling prone to substance abuse.

Mood & Affect: Depressed, anxious, flat affect



Client’s Progress: David attempted to use grounding techniques but reported limited success. Medication finally seemed to take the desired effect until the incident from last week. I am hopeful for the future.

Key Themes/Issues Discussed: David opened up about feeling hopeless and longing for his mother. He described a recent situation where a passerby grabbed his arm to ask what time it was, thus triggering a flashback to his kidnapping.

Interventions Applied: Cognitive-Behavioral Therapy was used to address negative thought patterns related to self-blame and worthlessness. Grounding exercises were revisited to help manage flashbacks. During the session, David was withdrawn and hesitant about therapy, but also hopeful when presented with new possibilities of medication.

Note on a margin: We were making progress, I’m not giving up on this boy.

Your name is David Szipka. When you were eleven two ghastly men plucked you from your mother’s arms and your careless childhood abruptly ended. Franklin Deagon, ex-biology teacher and a sadistic psychopath, abducted you with the aid of his partner in crime, Theodore, to trap you in a horrific labyrinth.



You weren’t their first victim, but you were their last. You’re the only one of many kidnapped children who escaped. Franklin lost his life, but it’s hard to tell if you truly got yours back…
