Patch 1.0.3d

- Found one source of crashes and fixed it (hopefully fixes random crashes in some levels)
- Changed the way monster and equipment details are displayed
- Fixed several bugs and issues
- Re-adjusted volumes for some monster attacks

Achievement fixes

Some achievements didn't trigger correctly on some cases. We fixed this and made a check when loading a save to give you any missing achievement
