Hotfix 1.2.8d

[p]This hotfix promotes build 1.2.8d, which has been sitting on NG+ beta branch, to default. It fixes:

• [p]Restored the old Book 2 resting conversations in elsewhere, which fixes the end of Natural Born Killer questline[/p]
• [p]Fixed icemaster not firing (it was trying to hit the Book 3 achievement Snow Day instead, since this part was ported from Book 3's engine)[/p]
• [p]Fixed Card Collector not triggering properly on NG+ mode[/p]
• [p]AchievementUI's 'report to steam' now does a better job of working out what achivements (like card collector) should be firing if they haven't yet[/p]
• [p]Updated debug UI (Ctrl+J) to have a working unlock all Card Backgrounds button
